Our providers participate with most insurance carriers. Below is a list of insurance plans that we currently are contracted to provide medical services. If your current insurance is not listed, please check with your insurance company or give our office a call at 678-705-7341 extension 265. We will be glad to discuss other financial arrangements with you so that we can participate in your care.
Participating Insurance Plans
All copayments and self-pay rates are due at the time of service. For pediatric patients, the parent or guardian that accompanies the patient to the visit is responsible for paying the amount due at the time of service.
If your insurance requires a referral to see a specialists, it is the patient’s responsibility to make sure one is on file. For Kaiser HMO patients, we are unable to file any claims that are not covered by a referral and the patient will be responsible for the visit.
Monthly statements are sent for outstanding balances. The statements are sent to the email we have on file. The e-statement will have a link to allow you to login and pay your bill online.
